The LAUNCH Creader crp123 scanner is the only one that has been able to read my JDM 2003 Prado 120 with the 5VZ engine. Tried several bluetooth scanners which work on other cars and also an Autel MS309 without any luck. The crp123 I had to configure to read Lexus codes instead of Toyota and also "Other region" instead of Japan. But it's a bit annoying having to have a scanner like that pluggedin with wire running across your lap while driving....

Bit the bullet and bought a wireless scanner from Autel, MaxiAP AP200 (https://www.amazon.com/dp/B07QD4GZW4)
This worked flawlessly on my 3.4L 2003 Japanese Prado 120 (5VZFE engine).
It reads a ton of different modules too. One downside is that I can't use the dongle with other software like Torque or DashCommand since it's solely for Autel software. Another is that it's designed for diagnostics, so no performance readings like 0-60, 1/4 mile etc
