Auto Transmission Clunk

Hi guys, my 97 3.4 prado will drop out of gear after heavy braking then go back into gear as you take off...
Any ideas?

Is the fluid level correct and oil in good nick, That would be my first check
Shifter linkage adjustment
Associated fuzes (not that I can think of any, but I have a diesel so maybe the pety has got some)
Gearbox/engine mounts (causing the shifter to be knocked into neutral perhaps)
