90 series Toyota Prado error code

I have a 1998 Toyota Prado 90 series, it is a 2.7 litre 4 cylinder petrol, it is a rzj95r model with a 3rz-fe engine. When you start the car it would stay on for about 5 seconds then turns off. However, if I start the car and give it a slight rev for about 1 min and take my foot off the accelerator it will not turn off once but the check engine stays on. I have a fault code 12. Please help?
