    For a Prado 150 / 2.8l engine.
    Does anyone have a list of all the conditions under which the ECU will put the vehicle into Limp Mode?.
    Also, the conditions under which the ECU will stop the vehicle?

    This is the 1GD-FTV failsafe list which I believe covers the answer to your question (one with engine stall will be a stop vehicle code)

      Excellent work.
      I notice there are no ABS and Transmission related failures.
      My vehicle wound up in limp mode after a transmission hose popped off at the radiator end. ATF got sprayed along the underside of the vehicle and onto the highway. Needed a new transmission.
      Is there a Legend that describes some of the terminology eg "Pass condition detected".

        There are fail safe lists for just about all systems on the vehicle but doing a quick scan of a few I couldn't find any that specified they put the engine into limp mode as most are about safety and protecting that system from preventable damage.

        Are you sure that your engine went into limp mode or did you just lose drive/gear selections due to insufficient trans fluid to maintain pressure in the solenoids (as I cant see any potential link to engine limp mode conditions in the failsafe list for the engine or transmission).

        For last point, if you look up the individual DTCs in the repair manual they give you the sensor being monitored and conditions for declaring a fault condition.
        So my understanding is those with "Pass condition detected" as the Failsafe Deactivation Condition will revert to normal operating mode and reset the DTC (move to a history code) when the monitored value goes above or below the applicable fault trigger threshold.

          Makes sense thanks. Looks like the actions taken by the algorithms vary widely depending on the situations.

          My trans went thru 2 phases.
          The first one occurred after the ATF hose popped off and dumped fluid. The vehicle lost power. When I stopped and restarted it seemed like it was stuck in 3rd.
          The second occurred after my Dealer cleaned up the mess and refilled the trans. Drove OK (harsh reverse gear change tho) for about 10Km then back to limping in a middle gear. New trans after that.

          I also followed the tribulations of a 150 driver who got stuck on a clay pan in the Simpson a couple of years back. I surmised he tried to rev his way out of the mud and boiled the ATF. The vehicle didn't lose any fluid but the gearbox shutdown and he was stuck on the clay pan overnight. The ATF cooled down and he got out in the morning. However the vehicle was in a limp mode. He managed to crawl the 100-200Km ish into Birdsville (don't know how he got over the big dunes). The mechanics couldn't do any quick fixes so he limped all the way to Adelaide!

            Funny the trans is not supposed to directly put the engine into limp mode at any temp (just modifies the shifting patterns and lock up operation when the TC temp goes over 130 degrees and also turns the dash light on at 150 - they revert to normal at 110 and 135 degrees respectively).
            My understanding is it will be the engine water and/or fuel temps going over threshold due to effect of the high ATF temps that will cause it to lose power or go into limp mode (should trigger a P1608 DTC which also sets flags as to which system/condition triggered the code in the Freeze Frame data).
            Problem with this is generally once it sets the MIL on, the conditions that triggered the issue need to have changed and DTC needs to be physically reset to revert to normal operation, or for some others that will happen when you cycle the ignition off as per the earlier list.

            AC60F failsafe list (sorry can't fix formatting but if you copy and paste info a spreadsheet may be able to sort)
            Transmission revolution sensor (NT) During a transmission revolution sensor (NT) malfunction, shift control is performed based on the transmission revolution sensor (SP2) signal.
            Transmission revolution sensor (SP2) During a transmission revolution sensor (SP2) malfunction, shift control is performed based on the wheel speed sensor signal.
            ATF temperature sensors (No. 1 and No. 2) During an ATF temperature sensor malfunction, up-shifting to 5th and 6th is prohibited.
            Shift solenoid valve SL1, SL2, SL3 and SL4 Stop the conduction of electric power to a failed solenoid and shift gears by turning on/off normal solenoids. If all solenoids are malfunctioning, gears will be selected through mechanical hydraulic circuits.
            Shift solenoid valve SLT During a shift solenoid valve SLT malfunction, the current to the shift solenoid valve is stopped.

            However, shifting is performed through normal clutch pressure control.
            Shift solenoid valve SLU During a shift solenoid valve SLU malfunction, the current to the shift solenoid valve is stopped.

            This stops lock-up control, thus decreasing fuel economy.

Many will most likely be same or similar to the 1GD version.
