I have a friend that has a 2007 d4d - just died at 155k, had injectors replaced 35k ago while still under extended new car warranty. Sounds very similar story. The photos of his pistons look just like yours. Fight with Toyota started today.

Put a hole in piston number 4. Hole through to gudgeon pin. Motor should be back tomorrow to start the refit. Bored 0.5 mm oversize.
Injectors have arrived from Baileys. New injectors lines. New engine mounts. Replaced thermostat.
Found a split in the catch can side valve. Provent will look at and may replace under warranty. Pity toyota wouldn't help with the motor. What were your injector feedback readings before the failure? I'm asking because my No 4 cylinder is very different to the rest with new Denso injectors.
Also, can you let me know that the readings are after the BDC injectors go in.
Did you use any additives with the previous injectors?2019 GXL, Bullbar, UHF, Redarc Brake Controller, Tow Bar, Secondary Fuel Filter

Can't get the readings before failure. We used genuine Denso at 150000 and the codes where done by a toyota dealer.
Motor should be back today and after I install the injectors etc will re install.
Never used any additives in fuel or oil.
Tunit have replied stating their chip cannot cause the failure but any engineer will tell you to extract more power from an engine the service life must be reduced proportionally.
I believe the chip had a effect on the engine although I never drove the engine hard. The chip didn't help as the increases rail pressure allowed for the gearbox to stay in fifth loading the motor at a lower rpm.

There s no absolute with these tickers.
We owners are definitely in the lottery.
Any chip especially one which increases rail pressure is not helping the issue.
It's very important to have a look at the pistons while fitting the new injectors, what's the point if damage has been done?
If I see notable damage to pistons i will advise the customer & they can decide if they want to change pistons as well.
Most pistons I look at while changing injectors in these do have some damage.
Very important to check those values every service. Not to what your TOYO says is ok....
Also it's a combination of the numbers, not the feedback values alone, the big picture & Klm's travelled.
Smoke? Rattle? Act on it don't ignore it.
Yes, you can still get caught out, but do everything you can to prevent it.
Such a wonderful vehicle & a pitty about one little problem that can be so BIG.

A couple of pics of the piston and helper.
Best looking TA I have ever had.
New engine in and running.
2 days to remove and "learn" how to. Those electrical harness are a real bugger to undo. 1 1/2 days to reassemble, injectors water pump etc and re install.
